Using the CORE SOW Data Collection Tool

The FY 2022-2023 CORE Data Collection Tool is an online website built to allow contracted vendors to report on their deliverables from their Scopes of Work. Each program will have a unique log-in which will give them access to their stated deliverables and allow the entry of their data. If you have questions or problems please e-mail corefunding@santacruzcounty.us and someone will get back to you shortly.

Please note: Each page has the Agency Name and Program Name. If you are given a log in that has the incorrect agency or program name please contact: corefunding@santacruzcounty.us.

Below is the description of the data entered into each section:

1. Unduplicated Participants
In this section you will enter the total unduplicated number of participants served by the program. This is a required response and the data entered must be a number.


2. Demographics
In this section you will enter the percentage of unduplicated participants served by the demographics of Age, Language, Ethnicity, Gender, and Location. If you program did not collect demographic information or the client information is unknown, please put that percentage in the Unknown category. These are required responses and the data entered must be a whole number (no decimal points). Please do not include the % symbol in your response.


3. How well did we provide services?
In this section you will provide the information on the required quality measure and the number of the unduplicated participants who were surveyed. This is a required response and the data entered must be a whole number (no decimal points), please do not include the % symbol in your response.


4. Is Anyone Better Off?
In this section you will provide information for your scope of work on the percentage of your participants who are better off. These are required responses and the data entered must be a whole number (no decimal points). Please do not include the % symbol in your response


5. Additional CORE Questions
In this section you will provide narrative responses to a series of questions. Responses are required except for the Optional Success Story. Please note responses are limited to 750 characters.

When you have completed the form, please click on the “submit” button. If you have incorrectly filled in any entry you will get an error message. Once you have submitted you are able to edit your responses by logging back into the form.
