Review No:1 1. Please indicate if the Medical Office will be licensed by the Department of Public Health. 2. Provide a site plan showing accessible parking and paths of travel on the site. 3. Please indicate occupancy type and occupant load for the area of work. Currently the plan shows occupancy types and occupant loads for the entire building. 4. Sheet A4 – Provide an exiting plan for the space to be improved. 5. Sheet A4 – Show accessible patient/customer counter areas that indicate height, width and clear access. The plan submitted shows counters at 42” in height. CBC 11B-904 6. Sheets M3 & E4 – This plan labels a clear exit area at the area called Billing. The area is labeled as 42” high counter area on Sheet A4. Resolve this conflict. 7. Sheet M3 - Provide a Mechanical plan by a licensed Mechanical Engineer or contractor. As there appears to be an alteration to the system it shall meet the requirements of the Green Building Code section 5.410.4 for the occupancy for testing and balancing of the system. Also Provide Energy form MCH-1-E and MCH-2-E which are required to be incorporated into a plan sheet. 8. Sheet M3 – The Mezzanine plan at the Nutritional Offices shows the supply and return air grilles being intersected by the new walls. Correct. 9. Sheets E4 & E5 - Provide a lighting plan that is designed by a licensed Electrical Engineer or contractor. Also provide Energy Code lighting forms NRCA- LT1-02-A, NRCA- LT1-03-A & NRCA- LT1-04-A which are required to be incorporated into a plan sheet. 10. Sheet P2 - The plumbing plan is required to be designed by a licensed mechanical Engineer or contractor.
