5. ORAL COMMUNICATIONS - Any person may address the Board during its Oral Communications period. Presentations must not exceed three (3) minutes in length, and individuals may speak only once during Oral Communications. All Oral Communications must be directed to an item not listed on today's Consent or Regular Agenda, and must be within the jurisdiction of the Board. Board members will not take actions or respond immediately to any Oral Communications presented, but may choose to follow up at a later time, either individually, or on a subsequent Board of Supervisor's Agenda. Award contract to Warehouse Direct Interiors in the amount of $15,300 for the Main Jail Booking Area Floor Replacement project; authorize the General Services Director to notify the contractor and sign the contract agreement and associated documents; authorize the General Services Director to approve change orders and encumber $1,530 to cover change order expenditures; and take related actions, as recommended by the Director of General Services
16.	Approve specifications (on file with the Clerk) for the Gasoline Dispensing Vapor Recovery project; direct the General Services Department to advertise for bids; and set the bid opening for 3:00 p.m. on December 8, 2004, in the General Services Department, as recommended by the Director of General Services
17.	Adopt resolutions regarding the medical plan contributions for employee groups under the Public Employees Medical and Hospital Care Act, as recommended by the Personnel Director
18.	Approve Memorandum of Understanding for the Middle Management and District Attorney/Child Support Attorney Associations and adopt resolution to implement various provisions for Middle Managers, District Attorney/Child Support Attorneys, and for unrepresented employees, as recommended by the Personnel Director
19.	Approve transfer of funds from liability reserves to liability claims, as recommended by the Personnel Director

HEALTH AND SOCIAL SERVICES

Authorize the Human Resources Agency (HRA) Director to execute a license agreement with the National Guard for use of the Santa Cruz Armory in an amount not-to-exceed $48,000; authorize the HRA Director to execute a contract with the Homeless Services Center for the provision of winter shelter facilities for fiscal year 2004-2005 in an amount not-to-exceed $50,445; authorize the HRA Director to execute a contract with the Salvation Army for the provision of winter shelter facilities for fiscal year 2004-2005 in an amount not-to-exceed $36,315; and adopt resolution accepting and appropriating unanticipated revenue in the amount of $6,886, as recommended by the Human Resources Agency Director

Approve gross pay adjustment in the amount of $747.12 for one Human Resources Agency employee, as recommended by the Human Resources Agency Director

LAND USE AND PUBLIC PROJECTS

22.	Schedule a public hearing on Tuesday, December 7, 2004 on the morning agenda at 9:00 a.m. or thereafter to consider the proposal to rezone three parcels from the Special Use (SU) zone district to the Timber Production (TP) zone district (Application Number 02-0431; Assessor's Parcel Numbers 089-021-15, -16, and -58 located at Hartman Creek Road, Boulder Creek), and take related actions, as recommended by the Planning Director
23.	Accept recommendation of the Planning Director relative to proposed amendments to the Grading and Riparian Corridor Protection ordinances regarding exempting certain activities in the Pajaro River from local permits and take related actions, as recommended by the Planning Director
24.	Defer award of contract for the Soquel-San Jose Road Storm Damage Repair project until January 11, 2005, as recommended by the Director of Public Works
25.	Approve Contract Change Order in the amount of $28,906 for the Buena Vista Landfill Entrance Facility Improvements, Phase I, Project Recovery Services; authorize the Chairperson of the Board to sign the change order on behalf of the County; and take related action, as recommended by the Director of Public Works
