PURPOSE:

 

     To describe the County policy on completing exit questionnaires and

     conducting exit interviews.

 

POLICY:

 

  I. As a standard practice, an exit interview will be conducted for all

     separations both for regular and extra-help employees.  During this

     exit interview, in exit questionnaire forms will be completed by the

     employee and the employee will check in any equipment assigned during

     the period of employment as appropriate.

 

 II. Each exiting employee will be referred to General Services to turn in

     keys and any parking permits.

 

III. Each operating department will develop procedures as to who conducts

     the exit interview.

 

PROCEDURE:

 

  I. Designated department staff will interview the separating employee

     using the Exit Interview Guide (PER5135).

 

 II. Results of the exit interview will be documented using the Santa Cruz

     County Employee Check-Out Form (PER1137B) and the Separation Report-

     Unemployment Insurance Questionnaire (PER5020).

 

III. The separating employee will sign and date the Separation Report-

     Unemployment Insurance Questionnaire (PER5020).

 

 IV. The separating employee will be given a copy of the Santa Cruz County

     Separating Employee Information Sheet (PER1137C).

 

  V. If appropriate, the separating employee will be provided with a PERS

     Retirement Information Packet (PER1137D)

 

 VI. Forms will be promptly forwarded to Personnel along with a Personnel

     Action Form to effect the separation.
