To identify the method of obtaining medical information to be used for employment related medical examinations.
POLICY:
The Medical History Questionnaire used for pre-employment medical exams and "baseline" medical examinations contains a release for medical records provision. Described below is a specific medical release form used in other employment related medical exams.
Occasionally a situation will arise where a department needs clarification as to whether or not an employee can perform the duties of their job class because of a medical condition. The department will need to clarify the employee's ability to do the work without endangering him/herself or others.
In some instances, there will be a need to remove the employee from the job until such clarification is received. See Civil Service Rule XI,D, Involuntary Leave of Absence Without Pay for Medical Reasons at Direction of Appointing Authority.
Should either of the above situations arise, departments should con tact the Employee Relations Division staff, who will discuss the circumstances with the department and the County's occupation health physician.
The department may then require the employee to sign a medical re lease form (PER142) so that the employee's medical records can be made available to and reviewed by the County's occupational health physician. The attached medical release form is used to obtain this information.
